Meaning
SMH is an internet initialism for “shaking my head” or “shake my head.” It is used as a brief reaction to something that seems foolish, frustrating, disappointing, absurd, or hard to believe, much as an actual head shake communicates disapproval without words.[1]

SMH is an initialism formed from the words “shaking my head,” a written shorthand for the familiar gesture of shaking one’s head in disbelief or disapproval. The exact creator and first spoken or typed use are unknown. A definition documented in February 2004 is the earliest widely cited online record, but that date marks a surviving record rather than necessarily the term’s birth. By 2012 and 2013, mainstream publications were explaining SMH as a common expression used in texts, on Twitter, and in online conversation. The related form SMDH means “shaking my damn head” and adds emphasis; it is not the expansion of SMH itself.
How to use it
SMH is informal and works best in texts, comments, group chats, gaming chat, and casual social posts. It can sound playful, dismissive, or genuinely annoyed depending on context. Because it often stands alone as a reaction, punctuation and surrounding words help show whether the tone is joking or critical. In professional or sensitive conversations, spell out the reaction instead of assuming the reader knows the initialism.
